Florid reactive periostitis - foot

Presentation

Throbbing pain and swelling at the forefoot and midfoot plantar aspect from 6 months ago.

Patient Data

Age: 30 years
Gender: Female

Ossified soft tissue mass lesions at the plantar aspect of the second metatarsal and digit.

Cortical sclerosis and periosteal reaction around the second digit phalanges in relation to the ossified soft tissue mass lesion.

There is a 13*23*24mm heterosignal ( low on T1 and high on PDfs ) mass lesion with peripheral scattered low signal areas of calcifications at the plantar surface of the second digit with adhesion to the flexor tendon.

Similar larger mass lesion measuring about 25*28*48mm at the plantar surface of midfoot with involvement of quadrants plantaris and flexor digiturom brevis muscles and bulging into space between the first and second metatarsal.

The histologic feature was compatible with florid reactive periostitis.

Case Discussion

The lesion of the 2nd digit was resected and histopathologic examination was compatible with florid reactive periostitis.
