Small bowel obstruction due to incarceration in femoral hernia

Two days of increasing abdominal pain, distension and now vomiting.

Patient Data

Age: 75 years
Gender: Male

There is an acute high grade small bowel obstruction. A funnel-shaped hernia sac containing a knuckle of small bowel protrudes posterior to the inguinal ligament into the right femoral space, with some compression of the femoral vein.

No signs of perforation or bowel ischemia.

Case Discussion

Feedback from surgery confirmed incarceration of the small bowel within a right femoral hernia. The hernia was successfully reduced and the small bowel was not ischemic.

These herniae are less common in men than women, but do tend to favor the right side.
